Procedure

1. Reporting a Work-Related Injury or Illness

Employee

1.1 Dial 911 if the injury requires immediate medical attention. 

1.2 Report a work-related accident, injury, or illness immediately to Security at (505) 224-3001 and immediate supervisor regardless of its severity or whether or not medical treatment is sought.

1.3 Go to an approved medical for treatment if the injury warrants medical attention and is not an emergency.

1.3.1 If the injury is a medical emergency, go to any treatment facility.

1.4 Complete a Notice of Accident Form within fifteen (15) days of the accident or injury and return to Human Resources. 

Supervisor

1.5 Submit the Notice of Accident Form within (15) days of the incident to Human Resources if the employee is unable to.

Security 

1.6 Complete and email the Incident Report to Human Resources and Risk Management

Human Resources Department 

1.7 Email the Notice of Accident Form to employee and supervisor upon notification of the incident. 

1.8 Submit the Employer's First Report of Injury or Illness report to the Worker’s Compensation Insurance provider within 72 hours of the incident. 

1.9 Email the Workers Compensation claim number to employee. 

2. Injuries Resulting in Days Away from Work

Employee

2.1 Submit an Absence Request in Workday, and indicate within the comments section that the absence is related to a Workers Compensation claim.  

Supervisor

2.2 Responsible for tracking absence related to Worker’s Compensation and will notify Human Resources if the employee misses seven (7) days. Non-consecutive days of absence are permissible.   

Human Resources

2.3 Place the employee on unpaid Family and Medical Leave (FML), when applicable. 

3. Workers' Compensation Payments

Payroll 

3.1 Provide the weekly wages report if requested by the workers compensation insurance carrier. 

3.2 The compensation benefits checks are either mailed directly to the injured worker or directly deposited in their bank account. 

4. Returning to Work

Employee 

4.1 Submit the anticipated return-to-work date accompanied by the physician’s return-to-work clearance documentation, which should outline any work restrictions.  

Human Resources 

4.2 Review the physician's return-to-work documentation provided by the employee. 

4.3 Notify the employee that the documentation has been received and they may return to work as scheduled. Human Resources will work directly with the supervisor regarding any return to work restrictions.  

4.4 Identify work restrictions which may or may not require a reasonable accommodation in compliance with The Americans' with Disabilities Act.
